Forms let you ask customers questions before their appointment, such as a health questionnaire, an intake form or a consent and waiver. Each customer's answers are saved against their record.
Forms are available to services businesses. They aren't shown for sports and facilities businesses.
Create a form
- Open More > Setup > Forms. You need the Create and edit services permission.
- Select Add.
- Under Basic information, enter a Form name your customers will recognise, such as "Health questionnaire". You can add an optional Introduction too.
- Under Services, choose which services need this form. A service can have more than one form.
- Choose when and how often customers get it (below).
- Add your Questions.
- Make sure Active is on, then Save.
Question types
Type | Use it for |
|---|---|
Short answer | A single line of text |
Long answer | A paragraph of text |
Single choice | Pick one option |
Checkboxes | Pick any number of options |
Dropdown | Pick one from a list |
Accept | Agree to terms or give consent |
Number | A number |
Date | Pick a date |
Mark a question as required if the customer must answer it. Consent ("Accept") questions are always required.
Only ask follow-up questions when they matter
A question can be shown only when an earlier answer calls for it. For example, ask "Please describe your allergy" only when the customer answers Yes to "Do you have any allergies?". Add a rule to the question that compares an earlier answer using is, is not, is answered or is left blank (or is ticked and is not ticked for consent boxes).
When customers get the form
Under When customers get the form, choose how it reaches them for online bookings:
- Ask during booking: the customer fills it in on your booking page before confirming.
- Email a link: they receive an email with a link to complete it.
Bookings made by your own staff always email the customer a link, since the customer isn't at the booking page.
Then choose How often:
- Once per customer: not asked again once the customer has completed it.
- Every appointment: asked each time they book.
See the answers
Open a customer from Customers and look at their Forms tab. You can also see a booking's forms on the booking itself. Answers are saved as they were when the customer completed the form, so editing the form later never changes what was already answered.
Good to know
- Deleting a form archives it. Past answers are kept.
- For a recurring booking made by staff, the form link is sent for the first appointment only.
- A guest who books without an account can't be recognised as a returning customer until after the booking, so they may be asked for a "once per customer" form again.
